【Full - Enrollment closed】 Certificate in Primary Healthcare for Social Workers

Introduction

As announced in the 2017 Policy Address, the government would allocate resources to promote district-based primary healthcare services so as to relieve the pressure on public hospitals. The social services sector has always been playing a significant role in primary healthcare in Hong Kong to promote community health and enhance the quality of life through professional services. The promotion of primary healthcare involves cross-sectoral and cross-disciplinary collaboration. Apart from providing services directly for the community, what are the other roles that social workers could play to enhance public awareness of health management?

The target participants of this programme are registered social workers. The programme will cover topics including: an overview of primary healthcare and its development in Hong Kong, health management knowledge, the roles of social services sector and social workers in primary healthcare, social work intervention and cross-sectoral collaboration. Apart from in-class learning, visitations and experience sharing sessions by guest speakers will be organized as part of the curriculum so as to facilitate participants to better understand the practice of primary healthcare services in different types of social service units. This programme will also establish a platform to strengthen the affiliation, experience sharing and mutual support network among the participants and the primary healthcare practitioners. 

Remarks: A portion of the programme fee will be subsidised by the Health Bureau. Participants who have completed the course successfully and awarded the certificate may apply to the Health Bureau for a reimbursement within 6 months of award. The final approval of the reimbursement is at the discretion of Health Bureau.
 

Objective

  • To learn about the development of primary healthcare and health management knowledge
  • To understand the roles of the social service sector and social workers in primary healthcare to enhance effectiveness of cross-sectoral collaboration
  • To enhance the professional knowledge and capabilities of social workers on primary healthcare service planning and delivery
  • To strengthen the affiliation, experience sharing and mutual support network among the participants and the primary healthcare practitioners

【Programme Subsidy】

  • Subsidy from Health Bureau (not applicable to applicants of "Small NGO Capacity Development Subsidy Programme"): 25% of the programme fee paid or $1,750, whichever is lesser, will be reimbursed to each participant upon successful completion of the programme and award of certificate.
  • Participants who have completed the course successfully and awarded the certificate may apply to the Health Bureau for a reimbursement within 6 months of award. The final approval of the reimbursement is at the discretion of Health Bureau. The application form for subsidy reimbursement will be distributed in the last session together with the programme completion certificate and payment receipt.
  • Applicants who have successfully applied for the "Small NGO Capacity Development Subsidy Programme" are not eligible to apply for the programme subsidy offered by the Health Bureau.

  • This certificate programme is not offered on a first come, first served basis. Screening of applications by HKCSS Institute for eligibility for taking the programme may apply.
  • ONLY online application and cheque payment are accepted for enrollment to this certificate programme (Cheque Payee: The Hong Kong Council of Social Service).
  • Upon submission of online application, please mail the programme fee cheque to HKCSS Institute within 5 working days. Application without cheque will be considered as incomplete application and no further screening on enrollment will be made. 
  • Application and cheque payment received after the early bird deadline (i.e. 16 Sept 2022) will not be entitled for the early bird fee.

Dr. TAM Yat Hung 譚一鴻醫生
Honorary Clinical Assistant Professor in the School of Public Health of the University of Hong Kong; Public Health Consultant and Medical Director of Shoebill Healthcare
Dr. Tam is a specialist in public health medicine. He graduated with his medical degree in the Chinese University of Hong Kong and further completed his master’s degrees in public health and buddhist studies, and Diploma in Family Medicine in Hong Kong, and infectious diseases in United Kingdom respectively.  He previously served as medical doctor in the former Centre for Health Protection and Senior Medical Officer in previous Primary Care Office, Department of Health, HKSAR. Currently, he is the Honorary Clinical Assistant Professor in the School of Public Health of the University of Hong Kong. He is working as both public health consultant and Medical Director of Shoebill Healthcare promoting vaccination, travel health and health management. He is also working as Network Medical Practitioner for a number of District Health Centre (DHC) and District Health Centre Express under the DHC Scheme. Dr. Tam has been active in the social welfare sector serving as advisor for NGOs and visiting doctors for residential care homes for the elderly and persons with disabilities. He is also a co-opted member of the Standing Committee on Policy Research & Advocacy, HKCSS. Dr. Tam is the Co-founder and Director of Health and Social Exchange in Hong Kong which is an NGO to provide a platform to support knowledge sharing and experience exchanges on specific topics between the health and social care sectors.
Ms. HUNG Anchor 熊德鳳女士
Vice-chair, Western Pacific Region, International Bureau of Epilepsy; Co-founder and Director of Health and Social Exchange in Hong Kong
Anchor Hung is a registered social worker with extensive experiences in health and rehabilitation services in Hong Kong. She obtained her Bachelor of Social Work at the Hong Kong Baptist University.  She also obtained her masters’ degree of disability studies in United Kingdom and public health in Hong Kong respectively. She was Senior Manager, District Health Centre Team, Primary Healthcare Office of former Food and Health Bureau on district health centres development. She also served in the senior management team in a few NGOs provide pioneering health and social services. She was Vice-Chair of Standing Committee on Policy Research & Advocacy and member of primary healthcare development group in HKCSS. She is volunteering as Vice-chair, Western Pacific Region, International Bureau of Epilepsy, and serving as honorary advisors for a number of NGOs. Anchor is also the co-founder and Director of Health and Social Exchange in Hong Kong which is an NGO to provide a platform to support knowledge and experience exchanges on specific topics between the health and social care sectors.

Peace Wong is the Chief Officer in the Policy Research and Advocacy Unit of the Hong Kong Council of Social Service, with poverty, social security, health, housing and ageing population as his main research foci. He has been recently working on how social-medical collaboration to promote the health of underprivileged groups and to reduce health inequity.

Dr. Lam Ching-choi is a specialist in paediatric and community medicine and is currently the Chief Executive Officer of Haven of Hope Christian Service. Under his leadership, Haven of Hope Christian Service is one of the pioneers in the provision of holistic care for the elderly in Hong Kong.

Dr. Lam is a non-official member of the Executive Council of the Government of the Hong Kong Special Administrative Region. He also serves as the Chairman of the Council for Sustainable Development to advise the Government on the related policies. He is also the Chairman of the Independent Commission Against Corruption Complaints Committee and the Supervisory Board Member & Nominating Committee Member of the HK Housing Society.

Dr. Lam was awarded the titles of Justice of the Peace (JP) and Silver Bauhinia Star (SBS) in 2003 and 2019 respectively. Apart from receiving Honorary Fellowship from Lingnan University in 2018, he was also given the Ageing Asia Global Ageing Influencer Award (Special Recognitions) for his devotion to public services and his influence on policy-making for the global ageing trend.

Dr. Ching is a Family Medicine specialist currently working part-time in the public sector. She is one of the founding board members of Health In Action since 2011. Dr. Ching has abundant experience in community health and development works in developing countries. She worked with various International NGOs, including MSF, Red Cross, World Vision, etc., in Africa and South-East Asia. She has obtained a Master's degree in Public Health in Developing Countries at London School of Hygiene and Tropical Medicine in 2013. Dr. Ching has also served for community health of the public healthcare system in Hong Kong for over 15 years. She has facilitated Health In Action to develop and build an innovative transdisciplinary primary healthcare model. Dr. Ching has stepped up as the Chairman of Health In Action since Sept 2020.
Mr. LAI Anthony 黎俊健先生

Mr. Lai is a Registered Social Worker and has a Master's Degree in Social Work. He has over 7 years of NGO working experience including project development, implementation, project evaluation and advocacy. He is currently responsible for community development and community partners engagement for health collaboration. He is also the line manager supervising social-medical collaboration projects for ethnic minorities, subdivided flat tenants, elementary workers, etc.
Mr. CHIU Cheuk Pong 趙焯邦先生
Physiotherapist

Chiu Cheuk Pong, local registered physiotherapist, practiced over 8 years in various local settings covering public, private and NGO sectors. Graduated from Master of public health in Hong Kong University in 2021 and started to work in Health In Action since then. Now working in "Jockey Club Healthy Neighbourhood Kitchen Project" as Physiotherapist, focusing in providing primary healthcare and the collaboration of health and social sectors to provide all-round services.
Dr. Leung Kit Ting , Joanne 梁潔婷博士
Lecturer at the Department of Social Work
The Chinese University of Hong Kong
Dr. Leung Kit Ting (PhD, Registered Social Worker) is a lecturer at the Department of Social Work at the Chinese University of Hong Kong (SWK/CUHK). She is an experienced social work practitioner in healthcare and multi-sectoral setting. Her expertise is in social work palliative care for cancer and non-cancer patients, end of life care and advanced care planning. She works on patients and families’ health-care decision-making dilemma, chronic disease management and patient support group service. She not only works on social work intervention with people having acute, chronic and terminal illnesses, but she also embraces the strength of achieving common goal and synergy of work in an inter-professional collaboration.

At present, she is the clinical practitioner and member at “Family and Group Practice and Research Center” (SWK/CUHK) and she is recognized as the Fellow of the Asian Academy of Family Therapy. Her expertise is to work with children and families with crisis of child maltreatment, parenting difficulties, rare diseases as well as adolescents’ mental health challenges, substance abuse, emotional and behavioral issues. Her research interests are ranging from family-based intervention, child protection, palliative care and social work in health care to program evaluation and practice-based research.
Mr. LAI Martin 黎嘉銘先生
Occupational Therapist I
Hong Kong Red Cross
Mr. Martin Lai is an Occupational Therapist I working in Hong Kong Red Cross, he also worked in Hong Kong Jockey Club Charities Trust to organise the Jockey Club Community eHealth Care Project. Mr. Lai is also Certified Dementia Care Planner (CDCP) in Hong Kong and Certified Dementia Practitioner (CDP) in the United States. Apart from clinical and managerial work, Mr. Lai is teaching in HKU School of Professional and Continuing Education as Adjunct Lecturer. Mr. Lai is also currently working on his doctorate in health science in the Hong Kong Polytechnic University. Mr. Lai is Fellow of Royal Society for Public Health in UK and member of the American Health Information Management Association (AHIMA).
